- Give your worker a Workers Claim Form
- Complete an Employers Report of Injury
- Send the worker's completed claim form and ACT medical certificate to us within seven business days of receipt. A case manager will assess the claim and come back to you with a liability decision.

- Complete an Employers Report of Injury
- Send the worker’s completed claim form and First Medical Certificate to us within three business days of receipt. A case manager will assess the claim and come back to you with a liability decision.
- Report notifiable injuries to WorkSafe Tasmania
- Tell us about the injury by completing a First Contact Form or contacting your local branch within three business days
- Give your worker a Notice of Right to Make a Workers Compensation Claim form within 14 days
- Give your worker a Workers Claim Form to complete and return to you with a Workers Compensation medical certificate
- Send the completed claim form and medical certificate to us within five business days of receipt. A case manager will assess the claim and come back to you with a liability decision.
